I would like to officially withdraw my student from Lutz Learning Center. I understand that this notice must be given prior to the 25th of the current month that the following month will be my child’s last month. I understand that if I don’t give notice within the time period stated, I forfeit my final deposit. I understand if I was awarded a loyalty scholarship and withdraw my child before the end of the school year, I forfeit the scholarship and my account will be adjusted accordingly. All other tuition and pre-paid fees are non-refundable including but not limited to; Registration, Field Trips, Supplies, Loyalty Application and pre-paid tuition.
